The Lead Case Manager's position will be in support of the following:

  • Active Duty Troops
  • Wounded Warrior Programs
  • Spouse & beneficiaries

This full time job will have the following typical schedule:

  • Monday - Friday, 40 hours/week
  • 8.5 or 9 hour shifts between 06:30am to 4:30pm
  • No federal holiday or on-call requirements

The ideal candidate will have the following experience:

  • Case Management
  • 3 years of supervisory experience
  • Occupational Health knowledge
  • Program Management
  • Familiarity with Military Culture/issues

Preferred candidate will have a BSN and experience in Project Management, Business Management, Human Resources, Nursing or Hospital Administration.

Provide advice and guidance to providers, registered nursing staff, senior executive staff and both clinical and administrative support staff regarding all Department of Defense (DoD) programs which affect coordination of care and utilization of health care services for active duty and non-active duty TRICARE Prime patients to include special programs relating to Wounded Warriors (including programs offered by the Department of Veterans Affairs) and active duty family members enrolled in the DoD Exceptional Family Member program, the DoD 1-1-1 Program, and other specialized programs for patients with both chronic and acute conditions. 


Document case management care plans in the Armed Forces Health Longitudinal Technology Application (AHLTA), Tri-Service Work Flow (TSWF), or MHS Genesis Electronic Health Record using BUMED and Defense Health Agency guidelines to ensure that high quality, comprehensive, cost effective and timely health care is provided.


Participate in monthly and quarterly peer documentation reviews.


Ensure patient education materials are offered to patients when applicable for topics; such as, pain management, alcohol abuse, diabetes, and other chronic conditions.


Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.  Assist in coordinating a multidisciplinary team to meet the health care needs, including medical and/or psychosocial management, of specified patients.


Must have one of the following:


(a) Certified Case Manager (CCM) issued by the Commission for Case Manager Certification

(b) Certified Disability Management Specialist (CDMS) issued by the Certification of Disability Management Specialists Commission

(c) Certified Rehabilitation Registered Nurse (CRRN) issued by the Association of Rehabilitation Nurses

(d) Certified Occupational Health Nurse (COHN) issued by the American Board for Occupational Health Nurses

(e) Advanced Certification in Continuity of Care (ACCC) issued by the National Board for Certification in Continuity of Care

(f) Certified Rehabilitation Counselor (CRC) issued by the Commission on Rehabilitation Counselor Certification

(g) Nurse Case Manager (RN-NCM) issued by the American Nurses Credentialing Center

(h) Care Manager Certified (CMC) issued by the National Academy of Certified Care Managers

Possess a minimum of 2 years full-time experience within the last 3 years as a registered nurse providing case management and obtain one of the certifications within 24 months of commencing service under this contract.


In addition, must possess:

  • Possess an Associate or Baccalaureate degree from an accredited nursing program
  • Valid Registered Nursing license from any US jurisdiction 
  • Basic Life Support certification (BLS) from an American Heart Association Program 
  • 3 year experience minimum as a Case Manager
