    NO Holidays

    NO on-call

    40 hrs per week

    Responsibilities

    • As a member of the Behavioral Health team, provides mental health clinical case
      management, care coordination and disease management.
    • This includes, but is not limited
      to the conditions of depression, anxiety disorders, tobacco cessation, weight management,
      TBI and PTSD: Monitors and facilitates adherence to treatments, provides education,
      coordinates care, and maintains data registries, performs full scope of clinical nursing in
      the execution of outpatient mental health care.

     

     

     

    Qualifications

    Must have:

    • Master's degree in Social Work (MSW).
    • Graduate from a School of Social Work fully accredited by the Council on Social Work
      Education (CSWE).
    • Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker.
    • Must have at least 12 months of direct hands-on experience in the last 24
      months with computer operations, basic word processing, data entry, and use of
      an automated medical record system.
    • Must have one year experience
      in the last three years in the evaluation and treatment of patients who have pain,
      psychological factors affecting their physical condition, or difficulty coping
      with medical problems.
    • Basic Life Support certification (BLS) from an American Heart Association Program
