    • As a member of the Behavioral Health team, provides mental health clinical case
      management, care coordination and disease management.
    • This includes, but is not limited
      to the conditions of depression, anxiety disorders, tobacco cessation, weight management,
      TBI and PTSD: Monitors and facilitates adherence to treatments, provides education,
      coordinates care, and maintains data registries, performs full scope of clinical nursing in
      the execution of outpatient mental health care.





    Must have:

    • Master's degree in Social Work (MSW).
    • Graduate from a School of Social Work fully accredited by the Council on Social Work
      Education (CSWE).
    • Possess certification as an Advanced Social Work Case Manager (C-ASWCM) by the National Association of Social Workers OR possess a minimum of three (3) years of experience within the preceding five (5) years as a social work case manager in a hospital-based setting performing medical crisis counseling and discharge planning.
    • Possess certification as a Certified Advanced Social Worker Case Manager (CASWCM).

    • Possess a minimum of two years postgraduate experience as a Social Worker Case Manager
    • Clinical experience within the past 2 years sufficient to demonstrate current clinical competency for the setting and procedures.
    • Basic Life Support certification (BLS) from an American Heart Association Program
